Because Tradeco Construction, together with all its subsidiaries, is considered a Large Employer for purposes of the Affordable Care Act (ACA), we are required to prepare and file Forms 1095-C (Employer-Provided Health Insurance Offer and Coverage)
It is the policy of Tradeco Construction NOT to furnish Forms 1095-C except by written request. Employees that are eligible to receive a Form 1095-C may request their own form by one of the following methods.

Your written request must include:
1. A statement simply requesting a copy of your Form 1095-C.
2. The Tax Year(s) for which you are requesting a form.
3. Your full name.
4. The last 4 digits of your Social Security Number.
5. The method you wish to receive the form:
a. Email (Be sure to include a valid email address.)
b. Postal Mail (Be sure to include your full mailing address.)
Upon receiving your written request, Tradeco will send the form(s) within 30 calendar days of your request or January 31, whichever is later.
